New Diver Application - For New Divers to PDC or divers that have been away for more than one year.
REMINDER: For Returning divers, registration with PDC will first require emailing to inform us of your interest to return and to make a schedule request... After receiving your schedule confirmation, mail in the Contract & Registration Form, the Medical Release Form and Fees.
New divers begin by submitting a New Diver Form... FYI: Returning divers in good standing are scheduled first, then openings are offered to new divers.
After your schedule has been cofirmed, you need to complete these two forms...they need to be signed and intialed in all areas to be accepted! You should be able to tab through the fields to complete the information...then print and sign.
